Many patients tell us the same thing at consultation: they feel confident about their face, then catch their neck in a photo or a video call and feel ten years older. The neck gives away age in a way the face often does not, and until recently the serious options for it were surgical. That has changed.

What Are Platysmal Bands?

The platysma is a thin, broad sheet of muscle that extends from the collarbone up to the jawline. Every time it contracts, through speaking, expression, tension, or simply habit, it can pull the neck into visible vertical bands. Over time, as the muscle works and the overlying skin thins with natural collagen loss, those bands become more defined at rest, changing the contour of the neck and softening the definition of the jawline.

This is a normal part of aging, and it is muscular in origin. That distinction matters, because a muscle-driven concern responds to a muscle-targeted treatment. This is exactly the mechanism BOTOX® Cosmetic was built on.


Why Does the Platysma Pull Your Face Down?

Your face is held in a constant balance between two groups of muscles: elevators, which lift, and depressors, which pull down. The platysma is one of the strongest depressors of the lower face, and with age it tends to win that tug of war.

What Is the Nefertiti Lift?

The Nefertiti lift is the name for using Botox along the jawline and the platysma to lift and define the jaw. It is named after Queen Nefertiti of ancient Egypt, whose famous sculpted portrait is known for its elegant, defined jawline. The technique was first described by Dr. Phillip Levy, who reported his experience in 130 patients, with noticeable re-contouring and elevation of the jawline and very high patient satisfaction (Levy, 2007).

In practice, the Nefertiti lift and Botox for neck bands target the same muscle. The difference is emphasis: neck band treatment focuses on the vertical cords, while the Nefertiti lift focuses on the platysma's pull along the jawline. How BOTOX® Cosmetic Works for the Neck

BOTOX® Cosmetic works by temporarily reducing the activity of targeted muscles. In the neck, a series of precisely placed injections relaxes the platysmal bands, allowing the neck to appear sleeker, firmer, and more contoured. Because the platysma also exerts a downward pull on the lower face, relaxing it can enhance jawline definition and soften the appearance of early jowls, creating harmony between the lower face and neck.

The FDA approved BOTOX® Cosmetic for moderate to severe platysma bands in October 2024, at doses of 26, 31, or 36 units depending on severity (Mraz et al., 2026). Individual results vary with anatomy, muscle strength, dose, and treatment plan, which your provider reviews with you before anything is injected.

What Does the Research Show?

In the Phase 3 randomized, placebo-controlled trial published in the Aesthetic Surgery Journal, 76.7% of patients treated with BOTOX® Cosmetic achieved at least a 1-grade improvement in their neck bands at Day 14, compared to 21.2% on placebo, and 41.0% achieved at least a 2-grade improvement, compared to 2.2% on placebo. Treated patients also reported better jawline definition, higher satisfaction, and less bother from their neck bands, and the treatment was well tolerated (Shridharani et al., 2024). A follow-up repeat treatment study found the effect lasts about 3 to 4 months per treatment cycle (Mraz et al., 2026).

Emerging research also explores what Heidi describes: a lift beyond the neck. In a study of 75 patients, neuromodulator injections along the lower face and jawline repositioned facial soft tissue upward within 30 days, with 3D scans showing more volume in the midface and less in the lower face (Hernandez et al., 2022). A follow-up study of 35 patients using the same approach found improvement in the tear trough and lid-cheek junction, which the researchers explained by the elevator muscles gaining a stronger upward pull once the platysma was relaxed (Germani et al., 2023). A separate study of 512 women with facial sagging also reported a very high degree of midface improvement after regional platysma injections (Zhou et al., 2019).

These findings come with important limits. The changes in the 75-patient study were promising but did not reach statistical significance, the under-eye study was small with no placebo group and used a different neurotoxin, the 512-patient study was conducted in Asian women using a different injection technique, and midface and under-eye improvement are not FDA-approved uses. We share this research because it explains what many patients notice, not as a promise. Your provider will tell you honestly what to expect for your face.

What to expect: the treatment timeline
Day 0
Consultation and treatment Your provider assesses your platysmal bands, overall neck anatomy, and goals, then performs a series of small, expertly placed injections. The appointment is quick and you return to normal activity the same day.

Who This Treatment Is For, and Who It Is Not

The best candidates are patients whose neck concern is driven primarily by muscle activity: visible vertical bands, a neck that looks tighter at rest than in motion, and early softening of the jawline connected to platysmal pull. Patients in the Sacramento region often ask about this treatment after noticing bands in video calls or photographs, and for the muscle-driven version of that concern, it is an excellent fit.

When this may not be the right treatment

BOTOX® Cosmetic relaxes muscle. It does not remove significant excess skin, tighten severe laxity, or reduce fat beneath the chin. Patients whose primary concern is loose or crepey neck skin may be better served by skin-directed treatments such as Morpheus8, or by a combination plan, and some anatomies are best referred for a surgical opinion. Treatment is also not appropriate during pregnancy or breastfeeding per our current clinical policy, and candidacy is always confirmed against your medical history at consultation. An honest assessment of which category your neck falls into is the first thing our providers establish, because the right treatment matters more than any single product.


Neck BOTOX® Cosmetic vs. Other Neck Options

Patients comparing options usually weigh three paths. Relaxing the muscle with BOTOX® Cosmetic is the right tool when bands are the concern: it is non-surgical, fast, and FDA-approved for exactly this use. Remodeling the skin with Morpheus8 is the stronger choice when texture, crepiness, or laxity dominates, because radiofrequency microneedling works on the skin itself rather than the muscle beneath it. Surgery remains the most complete answer for advanced laxity and significant excess skin, at the cost of incisions and recovery. Is Botox FDA-approved for the neck?

Yes. In October 2024, the FDA approved BOTOX® Cosmetic (onabotulinumtoxinA) for moderate to severe platysma bands, the vertical bands connecting the jaw and neck (Mraz et al., 2026). In the Phase 3 trial, 76.7% of treated patients improved by at least one grade at Day 14, compared to 21.2% on placebo (Shridharani et al., 2024).

What are platysmal bands and why do they appear?

Platysmal bands are the vertical cords that become visible on the neck when the platysma, a thin muscle running from the collarbone to the jawline, contracts. With age, repeated muscle activity and natural thinning of the overlying skin make the bands more defined, including at rest. Because the cause is muscular, a muscle-relaxing treatment addresses it at the source.

How fast will I see results from neck Botox?

Results begin to appear within days as the platysma relaxes, with full results typically visible at two weeks. Clinical studies demonstrated significant improvement as early as 14 days after treatment. Your provider may schedule a follow-up around that point to assess your result.

How long does neck Botox last?

Does treating the neck also improve the jawline and jowls?

It can. Can neck Botox lift the midface or improve under-eye hollows?

Emerging research suggests it may. Studies have reported midface improvement and better under-eye appearance after platysma injections, explained by the elevator muscles lifting more freely once the platysma relaxes (Germani et al., 2023; Hernandez et al., 2022; Zhou et al., 2019). These studies have limits, and midface and under-eye improvement are not FDA-approved uses, so results vary and are never guaranteed.

Is there downtime after neck Botox?

No significant downtime. There are no incisions, and most patients return to normal activity the same day. Mild tenderness or small marks at injection sites can occur and resolve quickly. Your provider will review simple aftercare guidance at your appointment.

How many units does neck Botox require?

Neck treatment generally requires more units than a single facial area because the platysma is a broad muscle treated at multiple points along each band. Exact dosing varies with band prominence, muscle strength, and whether facial areas are treated in the same appointment. Can neck Botox replace a neck lift?

No, and we will tell you honestly when it cannot. Neck BOTOX® Cosmetic relaxes muscle-driven bands. It does not remove significant excess skin or correct advanced laxity, which remain surgical territory. Treated patients also reported better jawline definition, higher satisfaction, and less bother from their neck bands, and the treatment was well tolerated (Shridharani et al., 2024). A follow-up repeat treatment study found the effect lasts about 3 to 4 months per treatment cycle (Mraz et al., 2026).

Emerging research also explores what Heidi describes: a lift beyond the neck. In a study of 75 patients, neuromodulator injections along the lower face and jawline repositioned facial soft tissue upward within 30 days, with 3D scans showing more volume in the midface and less in the lower face (Hernandez et al., 2022). A follow-up study of 35 patients using the same approach found improvement in the tear trough and lid-cheek junction, which the researchers explained by the elevator muscles gaining a stronger upward pull once the platysma was relaxed (Germani et al., 2023). A separate study of 512 women with facial sagging also reported a very high degree of midface improvement after regional platysma injections (Zhou et al., 2019).

These findings come with important limits. The changes in the 75-patient study were promising but did not reach statistical significance, the under-eye study was small with no placebo group and used a different neurotoxin, the 512-patient study was conducted in Asian women using a different injection technique, and midface and under-eye improvement are not FDA-approved uses. We share this research because it explains what many patients notice, not as a promise. Your provider will tell you honestly what to expect for your face.

Who This Treatment Is For, and Who It Is Not

The best candidates are patients whose neck concern is driven primarily by muscle activity: visible vertical bands, a neck that looks tighter at rest than in motion, and early softening of the jawline connected to platysmal pull. Patients in the Sacramento region often ask about this treatment after noticing bands in video calls or photographs, and for the muscle-driven version of that concern, it is an excellent fit.

When this may not be the right treatment

BOTOX® Cosmetic relaxes muscle. It does not remove significant excess skin, tighten severe laxity, or reduce fat beneath the chin. Patients whose primary concern is loose or crepey neck skin may be better served by skin-directed treatments such as Morpheus8, or by a combination plan, and some anatomies are best referred for a surgical opinion. Treatment is also not appropriate during pregnancy or breastfeeding per our current clinical policy, and candidacy is always confirmed against your medical history at consultation. An honest assessment of which category your neck falls into is the first thing our providers establish, because the right treatment matters more than any single product.

Patients comparing options usually weigh three paths. Relaxing the muscle with BOTOX® Cosmetic is the right tool when bands are the concern: it is non-surgical, fast, and FDA-approved for exactly this use. Remodeling the skin with Morpheus8 is the stronger choice when texture, crepiness, or laxity dominates, because radiofrequency microneedling works on the skin itself rather than the muscle beneath it. Is Botox FDA-approved for the neck?

Yes. In October 2024, the FDA approved BOTOX® Cosmetic (onabotulinumtoxinA) for moderate to severe platysma bands, the vertical bands connecting the jaw and neck (Mraz et al., 2026). In the Phase 3 trial, 76.7% of treated patients improved by at least one grade at Day 14, compared to 21.2% on placebo (Shridharani et al., 2024).

What are platysmal bands and why do they appear?

Platysmal bands are the vertical cords that become visible on the neck when the platysma, a thin muscle running from the collarbone to the jawline, contracts. With age, repeated muscle activity and natural thinning of the overlying skin make the bands more defined, including at rest. Because the cause is muscular, a muscle-relaxing treatment addresses it at the source.

How fast will I see results from neck Botox?

Results begin to appear within days as the platysma relaxes, with full results typically visible at two weeks. Clinical studies demonstrated significant improvement as early as 14 days after treatment. Your provider may schedule a follow-up around that point to assess your result.

How long does neck Botox last?

A Phase 3 repeat treatment study found results last about 3 to 4 months per treatment cycle (Mraz et al., 2026). Duration varies with anatomy, dose, and muscle strength, so your provider will recommend a maintenance schedule based on how your platysma responds to your first treatment.

Does treating the neck also improve the jawline and jowls?

It can. The degree of improvement depends on how much of your lower-face change is driven by platysmal pull versus skin laxity or volume loss, which your provider evaluates at consultation.

What is a Nefertiti lift?

A Nefertiti lift uses Botox along the jawline and platysma muscle to lift and define the jaw. It is named after Queen Nefertiti of ancient Egypt, known for her defined jawline, and was first described by Dr. Phillip Levy (Levy, 2007). It targets the same muscle as FDA-approved Botox for neck bands, with the focus on the jawline.

Can neck Botox lift the midface or improve under-eye hollows?

Emerging research suggests it may. Studies have reported midface improvement and better under-eye appearance after platysma injections, explained by the elevator muscles lifting more freely once the platysma relaxes (Germani et al., 2023; Hernandez et al., 2022; Zhou et al., 2019). These studies have limits, and midface and under-eye improvement are not FDA-approved uses, so results vary and are never guaranteed.

Is there downtime after neck Botox?

No significant downtime. There are no incisions, and most patients return to normal activity the same day. Mild tenderness or small marks at injection sites can occur and resolve quickly. Your provider will review simple aftercare guidance at your appointment.

How many units does neck Botox require?

Neck treatment generally requires more units than a single facial area because the platysma is a broad muscle treated at multiple points along each band. Exact dosing varies with band prominence, muscle strength, and whether facial areas are treated in the same appointment.
